Thermal water for joints and back pain: what 125 European baths claim, and what is actually in the water

Search for a thermal bath with a bad back and you will find the same sentence everywhere: the water helps with joint and rheumatic complaints. It is rarely clear who is saying it. Sometimes it is a national health authority that has tested the spring and issued a certificate. Sometimes it is a marketing agency that has never seen the analysis.

This article separates the two. We counted every venue in the catalogue whose operator states the water is used for musculoskeletal, rheumatic or post-injury complaints, and then looked at what those operators publish about the water itself.

What we counted

125 thermal baths in twelve countries. The claim breaks down like this — a bath can state more than one, so the parts add to more than the whole:

Alongside those, the same 125 baths often state other uses: 39 circulatory, 32 skin, 19 gynaecological, 14 respiratory, 8 digestive. A bath that claims eight different uses is telling you something different from one that claims two, and it is worth noticing which you are reading.

The distribution is not a map of where thermal water exists. It is a map of where operators write down what their water is for — which is a matter of national regulation and habit. Hungary and Slovakia have long-standing registers of medicinal water and a public used to reading them. Italy has plenty of thermal water and says far less about it in this particular way.

The one distinction that matters

67 of the 125 hold an official medicinal-water certification. That is a legal status granted by a national authority, not a description an operator may choose for itself.

In Hungary the designation is gyógyvíz, granted by the national public health authority. A bath may not use the word without it.

In Germany the parallel term is Heilwasser, approved under German medicines law.

In Portugal and Spain the equivalent is a declaration of água mineromedicinal — mineral-medicinal water.

What the certificate attests is that the water has a stable, measured composition that meets a defined standard. It is a statement about the water, not a prescription. The remaining 58 baths may have water that is chemically indistinguishable — they simply have not been through the process, or their country does not run one.

What is actually in the water

Across the 125 baths, the dominant constituents their own analyses name:

Sulphur leads, and if you have ever walked into a thermal bath and wondered about the smell, that is it — hydrogen sulphide, detectable by the human nose at concentrations far below anything that matters chemically.

Only 18 of the 125 publish a total mineralisation figure at all, which is the number that would let you compare one water with another. Among those that do, the spread runs from 594 mg per litre to 11,000 — a factor of eighteen between the weakest and the strongest, which is the difference between a light mineral water and a brine. Naturtherme Bedernau in Bavaria sits at the bottom of that range; Zsóry Gyógy- és Strandfürdő at Mezőkövesd publishes 3,890 and Túrkeve on the Hungarian Great Plain 4,192.

Temperature: colder than people expect

67 of the baths publish a pool temperature. The median is 37 °C — body temperature, the point at which water stops feeling like anything at all. The range runs from 26 °C to 45 °C.

The pool temperature is usually not the spring temperature. Water arrives hot and is cooled:

Les Bains de Lavey in Switzerland draws water at 67 °C from 1,600 m and runs its pools at 45 °C — the warmest figure in this set, matched only by Tölgyes Strand in Hungary.

Schwaben Therme at Aulendorf takes 55 °C water from 1,200 m and holds its pools between 28 and 40 °C.

Virágfürdő at Kaposvár draws 56 °C from a 1,050 m well and cools it to between 28 and 38 °C.

At the other end, Kabai Városi Gyógyfürdő runs at 26 °C — cool enough that most visitors would call it a swimming pool, and certified medicinal all the same.

How to read a claim before you travel

  1. Look for the legal term, not the adjective. Gyógyvíz, Heilwasser, mineromedicinal are regulated. "Healing", "therapeutic" and "wellness" are not.
  2. Ask what the water contains. A bath that publishes its analysis has one. A bath that only publishes adjectives may still be excellent, but you are reading its marketing rather than its water.
  3. Check the pool temperature separately from the spring temperature. A 60 °C spring tells you about geology. The pool is what you get in.
  4. Notice how many uses are claimed. Two or three, tied to the composition, is a different kind of statement from a list of nine.
  5. Check when the bath last confirmed its own figures. Prices and hours change every season; analyses do not, but the page they sit on can be a decade old.

What this article does not tell you

Whether any of it works. We have not examined a single visitor, and no count of baths is evidence about a body. Balneotherapy for musculoskeletal complaints has a real research literature, it is genuinely contested, and it is not what this catalogue can speak to.

What we can say precisely is what 125 operators state, what their published analyses contain, and which of them hold a certificate that somebody with authority issued. If you are choosing between two baths for a bad back, that is the ground you can actually stand on — and the rest is a conversation for a doctor who has met you.
